Medical Sliding Fee Calculator
Using the form below you can calculate the fee you would need to pay for your medical visits. Different fee structure applies to dentist visits. Therefore, if you need to see a dentist please call and make an appointment. An income-based sliding fee program is available for those individuals/families without insurance coverage.
There is a $20 minimum medical fee. There is a $55 minimum dental fee.
Sliding fee is based on the federal poverty guidelines (01/24/2009)
