Sliding Fee Calculator
Using the form below you can calculate the fee you would need to pay for your medical visits. Different fee structure applies to dentist visits. Therefore, if you need to see a dentist please call and make an appointment. An income-based sliding fee program is available for those individuals/families without insurance coverage.
There is a $20 minimum on the medical fee and $55 minimum on the dental fee (Patients are responsible for all Lab charges at 100%)
Sliding fee is based on the federal poverty guidelines (01/24/2007)
