Council Bluffs Community Health Center provides comprehensive medical services to it patients. Our provider staff includes Family Practice Physicians and a physician's assistant.
Dental
Our provider staff includes a Dentist and Dental Hygienist. CBCHC offers preventative dental services including:
- Exams
- X-rays
- Cleaning
- Extractions
- Fillings
- Crowns
- Dentures (limited)
Pharmacy Services
Council Bluffs Community Health Center offers on site pharmacy services for many prescriptions and works with Hope Medical Outreach to provide low-cost prescriptions for many others. Patients who are eligible for the sliding fee program receive their medications at a significantly reduced rate. CBCHC participates in the 340B Drug Discount Program, sponsored by the federal government. Savings to our patients for their medications range from 30% to 80%.
Medication Assistance Program
CBCHC offers several prescription assistance options to its uninsured and underinsured patients. Through various pharmaceutical companies, Hope Medical Outreach completes applications for patients to obtain their medication at an affordable cost.
Radiology (X-ray) Services
CBCHC has a certified X-ray technician to provide limited X-ray services.
Health Disparities Collaborative
CBCHC, as leaders in managing quality improvement, and moving toward the elimination of health disparities in our community, has implemented key elements to improve health outcomes among patients with chronic diseases. Currently, CBCHC has developed chronic disease management programs. These programs address patients diagnosed with diabetes and cardiovascular disease.
